One Stop Service Center
When you are not sure where to start, Community of Care is here to help. Our Care Coordinator/Social Worker provides one-on-one support to older adults and families in rural Cass County as they navigate healthcare, benefits, assistance programs, and community resources.

Whether you need help completing paperwork, coordinating care, or finding services that make day-to-day life easier, we are here for you. All services are provided free of charge.

Resources, Benefits, & Paperwork
We can help you find information and referrals for financial assistance, equipment needs, housing, in-home services, medical needs, nutrition, transportation, and other everyday needs. Our trained team members can also help complete applications and paperwork for assistance programs, including fuel, food, prescription, and Medicare-related help.

Medicare Counseling 
Our staff are trained and certified as State Health Insurance Counseling (SHIC) counselors through the North Dakota Department of Insurance. We offer confidential help with Medicare, including Part D drug plans, Medicaid, other health insurance, and long-term care insurance. We can also help you complete paperwork and understand statements from hospital stays and clinic visits.

Care Coordination
When needs are more complex, our Care Coordinator can help arrange services and medical appointments, communicate with family members, assist with day-to-day finances, and coordinate next steps. We work with you to help make sure your support is organized and connected.

Advance Care Planning
Planning ahead helps you share your healthcare wishes with the people who matter most. Our Care Coordinator and Faith Community Nurses work together to help clients complete an Advance Care Plan and submit it to healthcare facilities.

Home Visits & Social Connection
For homebound older adults, staff can make home visits. We also create opportunities for older adults to connect with others through social events and activities in their community.
