Faith Community Nurse Program
Community of Care’s Faith Community Nurses are licensed registered nurses who provide preventive health support for older adults throughout rural Cass County. They help older adults remain healthy, connected, and independent in the homes and communities they love.
The role of the Faith Community Nurses is to assess, educate, and refer- helping older adults understand health concerns, physician instructions, and medications; providing practical health education; and connecting clients with appropriate healthcare providers and community resources.
Assess, Educate & Refer
Faith Community Nurses provide personalized visits and support for older adults who have questions or concerns about their health. They listen, assess needs, offer health education, and help clients identify appropriate next steps and referrals.
Outreach Visits & Health Support
For older adults who may have difficulty getting out, Faith Community Nurses can provide outreach visits. They help clients better understand health conditions, medications, provider instructions, and resources that may support their health and independence.
Wellness, Prevention & Education
Our nurses facilitate opportunities that support healthy aging, including Bone Builder exercise classes, blood pressure screenings, wellness education, facilitation of local Health Cabinets, and community health events. These programs help older adults stay active, informed, and connected to their communities.

Advance Care Planning
Planning ahead helps you share your healthcare wishes with the people who matter most. Faith Community Nurses work alongside Community of Care’s Care Coordinator to help clients complete an Advance Care Plan and submit it to healthcare facilities.
