Medicare Open Enrollment is October 15 through December 7. Community of Care can help you review your Medicare Part D prescription drug plan and explore your options for the coming year.

  • Kindred Lutheran Church — November 2
  • Horace Lutheran Church — November 12 and 13

Please bring:

  • Your medication bottles or an up-to-date medication list
  • Your Medicare card
  • The name of your preferred pharmacy
