How Your Gift Helps
Every gift to Community of Care helps older adults in rural Cass County stay connected, independent, and at home in the communities they love.

What a Gift Can Make Possible
A gift to Community of Care can help make moments like these possible:

  • An older adult gets a ride to an important medical appointment when transportation would otherwise be difficult.
  • A caregiver receives guidance, reassurance, and help navigating the next step.
  • A neighbor sits down with a trained counselor to better understand Medicare Part D options and prescription-drug costs.
  • An older adult visits with a Faith Community Nurse about a health concern.
  • A local resident finds strength, connection, and encouragement through wellness and exercise programs.
  • A person receives help completing paperwork or connecting with resources that support living safely at home.

Behind each service is a person, a family, and a rural community made stronger by having support close by.

In 2025, Community of Care:

  • Served 711 unique clients throughout the year.
  • Provided 309 rides to medical appointments.
  • Helped 365 clients during Medicare Part D open enrollment.
  • Helped clients save a combined $402,228 on prescription-drug costs during Medicare Part D open enrollment.
